Description

The Special Education Transitions Facilitator is responsible for supporting the instruction of direct services to students (ages 14-22) with disabilities. This includes understanding and being able to communicate individually or in groups and to facilitate learning of topics related to transitioning to adulthood.

Responsibilities

Supporting a caseload of 3-4 high schools across wake county.
Creating lesson materials, data entry, billing and classroom support.

Required Education / Experience

Associates Degree in Psychology, Special Education, Rehabilitation or a related human services field preferred.
Experience working with students with developmental disabilities. Equivalent experience will be considered in lieu of education.

Minimum Qualifications

Must have valid NC Driver’s License and driving record that adheres to Agency standards.
Must be able to work in various schools across the county.
Must pass background check and drug screening.

Physical Requirements

Must be able to lift 10 - 25 lbs.
Must be able to handle sitting and standing for extended periods of time

Compensation

$18.00 - $18.50 per hour
